The reinvention of micro needling

Cosmetic doctors swear by it as an alternative to unreliable dermal fillers; patients rave about the results; it’s proven to deliver incredible medical and cosmetic results. 

What are we talking about? Microneedling. This is the story of how a 90s favourite beauty treatment got a fancy facelift for 2020.

As we think of it today, microneedling became a cosmetic phenomenon in the mid 90s in America, when a skin rejuvenation expert used a needle stamp to treat scars and then lines and wrinkles. This was initially known as collagen induction. 

The needles were administered using manual dermarollers that, while still used today, don’t offer the same precise application of the mechanical microneedling pens that arrived later down the line. 

The advent of at-home dermarollers seemed positive but brought about cases of women damaging their skin, by using low-quality devices or incorrect needle settings causing infections. 

So whether you choose to use at-home devices or seek the specialist treatments available in clinics only, how does microneedling actually work?

Microneedling devices differ but all involve small needles being pushed or rolled over the face and neck area to create controlled punctures, which force your skin into repair mode. 

When your skin starts repairing itself it turbocharges your body’s natural ability to produce collagen and elastin, which provides that firm plumpness. It also helps improve skin texture, reduce the appearance of scars and stretch marks and creates a more youthful skin tone. 

Those with a fear of needles shouldn’t discard this technique straight away as, depending on the device used, the microneedle length can vary from 0.5 to 3.0 mm, which is incredibly tiny compared to a traditional needle.  

The number of treatments depends on the skin condition, but typically you’re looking at a minimum of 3 treatments with 4-6 week intervals with maintenance treatments every 4-6 months at a clinic.

So how has this type of skin rejuvenation treatment evolved? UK aesthetic experts agree that at-home dermarollers, made popular in the 2000’s are not as effective or safe as in clinic microneedling treatments. 

While dermarollers can pull and tear the skin, microneedling pens offer more precise and controlled rejuvenation.

For someone simply looking to plump and add luminosity to their skin it could be used as a one-off treatment; SkinPen could most definitely be an advanced specialist treatment. But for those skins that have more serious anti-ageing concerns a course of three would be recommended.

Skincare brand bids to become the ‘world’s most sustainable professional brand’

Skincare brand Medik8 has announced that every product manufactured by the brand now uses 100% post-consumer recycled plastic. 

This is in line with the brand’s wider campaign surrounding the Professional Refresh (launching October 2019), positioning Medik8 as “The World’s Most Sustainable Professional Brand.”

These commitments towards ensuring a more environmentally friendly product and minimising the brand’s carbon footprint are just a few of the many initiatives within Medik8’s sustainability crusade, making the above statement more than fitting. 

From September 2019, every professional Medik8 product in a plastic bottle will be made from 100% Post-Consumer Recycled (PCR) material. This means not only are they recyclable, but they are also MADE of 100% recycled material. 

Medik8 uses plastic derived from former drinks bottles that would otherwisehave just gone to landfill; taking centuries to degrade. This PCR material is primarily sourced from the UK to minimise our carbon footprint.

Introducing Ozone Autohemotherapy

Product in Focus: Nithya Injectable Collagen

Nithya: the basics

From the age of 25, we lose up to 1.5% of collagen from our skin each year. The resulting appearance of wrinkles due to the loss of our natural collagen can be mitigated by replenishing it and this can now be achieved through treatment with injectable collagen, a product designed for the regeneration and reconstruction of tissues.

Nithya collagen is produced from equine Achilles tendon tissue in a sterile environment. Nithya is a Type I collagen powder which stimulates the production of new fibroblasts to create native Type III collagen during the immunodefense responses triggered by the cell regeneration process known as mechanotransduction, a process that is fundamental in cosmetic medicine and anti-aging treatments.

When the collagen is native it reduces the presence of free telopeptides, a source of allergenicity, almost to zero. It can be defined native because it preserves the same physical and chemical characteristics typical of collagen in the living organism (same sequence and composition of amino acids, same molecular structure); the collagen protein retains its native triple helix structure and there are no free telopeptides.

The equine collagen of Nithya passed all the cytotoxicity, allergenic and skin irritability tests. The product is safe and doesn’t have any contraindications.

Nithya produces the optimal conditions to restore connective tissue. It supplements dermal bio-revitalization and assists the regeneration of connective tissue in the dermis providing perfect conditions for the physiological neo-formation of collagen and can be used for body and facial chrono- and photo-aging treatments.

nithya-picPackaged in powder form, sterile, pyrogen-free and for single use, Nithya must be placed in suspension immediately before use in WFI (Water for Injection) and Lidocaine at a ratio of a 70mg vial of collagen to 4.5ml of WFI and 0.5ml of Lidocaine. Once in suspension, the product should be used immediately and any remaining following the treatment should be discarded.

Using the appropriate size needles from 32G x 2 mm up to 30G x 4 mm, the product is administered through intradermal infiltration by medical practitioners licensed to carry out such treatments. The technical skill of the practitioner is key to the success of the treatment so it is recommended that the practitioner receives specific training on the mesotherapy injection technique.

Nithya bears the CE 0373 mark and is manufactured in Italy by Euroresearch. Used in combination with lidocaine, the procedure is virtually painless and is the only commercially available Type I collagen intended for aesthetic use. Equine collagen, produced by Euroresearch, has been used in pad form for more than 20 years for its healing property on skin ulcers, open wounds, scars and pressure sores.

Equine collagen is FDA approved for wound therapy under the brand name BIOPAD. BIOPAD is produced by the same manufacture as Nithya and uses the same collagen source.

What are the advantages to using Nithya equine collagen compared to bovine collagen?

Studies have shown that 3% of the population are allergic to bovine collagen whereas equine collagen has not shown this level of adverse reaction and in fact, no cases of allergic reaction have been reported. Equine collagen is extracted from the tendon whereas bovine collagen is extracted from the hide.

This results in the equine collagen being a higher quality and purer product when compared with bovine collagen. In addition, it is Type III collagen that is extracted from the bovine hide rather than the Type I from the equine tendon. By using Type I to stimulate production of Type III collagen, the body has more control over production and distribution leading to better and longer lasting results.

The allergic reactions caused by bovine collagen are connected to the source and the extraction process. The collagen is usually extracted from the bovine derma which is a less pure source compared to the tendon, therefore during the extraction process the collagen fibre is cut and consequently this frees polypeptides which can cause allergic reactions to bovine collagen.

Equine collagen is a much more popular choice compared to bovine collagen because the amino acid sequence is more like the human one which makes it work much better with the human body and enhances the results from the collagen.

What are the differences between Type I and Type III collagen?

Type I is the most numerous in number among all collagen types. It can be found in bones, skin and intervertebral disks. It can also be found in scar tissue.

Type III is the second most abundant collagen in the body. It is closely related to collagen Type I in the terms of location and manner of synthesis. It is primarily located in the intestinal walls, muscles and blood vessels, although it can also be found with collagen Type I. Type I and Type III can be mixed together to create supplements for the skin, bone and muscle.

How do you know the animals aren’t infected?

Nithya equine collagen is a bi-product of an established industry. The animals are slaughtered for human consumption and, as with any animal entering the human food chain, are subject to rigorous checks, tests and strict processes. All horses destined for human consumption will have been monitored using a ‘passport’ scheme which will have a complete record of any drugs that have been administered, any indications of recent medications and/or drugs being administered that might render the animal unfit for human consumption.

The treatment was also recently shown, in a peer-review study published in the Journal of Drugs and Dermatology, that it treats more fat faster—and over a much larger area—than its competitors on the market.

They are the only radiofrequency devices on the UK aesthetic market that allow for non-contact, hands-free treatment of subcutaneous fat.

The multi-centre study, conducted by Drs. Nicole Hayre, Peter Jenkin and Melanie Palm in the US, sought to compare clinical outcomes of a non-invasive selective radiofrequency (RF) field device (BTL Vanquish ME) with its predecessor (BTL Vanquish).

A total of 36 subjects with BMIs under 30 but above 24, were randomly assigned to be treated in Group A (BTL Vanquish ME) or Group B (Vanquish) to obtain a side-by-side comparison of the devices’ efficacies. Each subject received four, once-weekly, 45-minute treatments with their group’s determined device. Measurements of subjects’ abdominal fat were taken prior to the first treatment and again four weeks after finishing the final treatment.

Subjects had also been instructed to maintain their usual diet and lifestyle throughout the duration of the study.

Abdominal fat layer thickness reduction at 30 days post treatment was the primary outcome measurement in the study as measured by ultrasound. The degree in fat layer thickness between Group A and B was statistically significant (p<0.008), and data showed that fat for Group A (BTL Vanquish ME) and Group B (BTL Vanquish) was reduced by 29.5% and 15.2%, respectively.

That represents a 53% higher reduction of abdominal fat layer thickness with Group A than for those in Group B. Every subject in the study showed a reduction in the abdominal fat layer thickness.

–       Group A: Reduction ranged from 2.0 to 6.6 mm (average of 4.17 ±1.42 mm)

–       Group B: Reduction ranged from .07 to 10.4 mm (average of 2.72 ± 2.21 mm)

The standard deviation of ultrasound measurements in Groups A and B were 1.42 mm and 2.21 mm, respectively—and this reduction in standard deviation of fat reduction measurements in Group A vs Group B is evidence that the BTL Vanquish ME™ provides more consistent performance.

Finally, using the measured reduction in fat layer thickness and the treatment surface area of the applicator (2100 cm2or 325.5 square inches), a reduction of 0.876 liter (0.23 liquid gallon) and 0.571 liter (0.15 liquid gallon) of fat were calculated for Groups A and B, respectively. (Volume lost = fat layer thickness reduction x effective area or spot size of the applicator (2100 cm2; 325.5 square inches).

Further speaking to its uniqueness in the market, the BTL Vanquish ME device holds three patents: One, granted in 2013, protects the non-contact method of adipose tissue treatment.

The second, issued in September 2016, is for the feedback power control device for non-invasive treatment of skin and human tissue.

The third—issued in October—refers to a new method for treating subcutaneous tissue that positions one or more applicators adjacent to the skin of a patient, but not touching the skin or in need of continual movement by the applicator. These applicators, which transmit electromagnetic energy into the skin where it can be used for subcutaneous tissue treatment, may also be used for tightening the skin and for remodelling collagen tissue in the subcutaneous tissue.

Additionally, airflow can be provided between the applicator and the skin by positioning an air mover (attached or made part of the applicator) close to the skin.
